Notice of Privacy Practices
How medical information about you may be used and disclosed, and how you can get access to this information.
Please review this notice carefully
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Mission Oncology Partners is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
We may use and share your health information without your written authorization for the following purposes.
For treatment. Your radiation oncologist, therapists, dosimetrists, physicists and nurses use your information to plan and deliver your care, and we share it with the physicians who referred you and with others involved in your treatment.
For payment. We use your information to bill and collect from you, your insurer, or another payer — which can include confirming coverage and obtaining prior authorization.
For health care operations. We use it to run the practice: quality review, staff training and evaluation, accreditation, licensing, audits and business planning.
Appointment reminders and health information. We may contact you to remind you of an appointment, or to tell you about treatment alternatives or health-related services that may interest you.
Business associates. Some services are performed by contractors — billing, transcription, IT. Each is required by written agreement to protect your information as we do.
As required or permitted by law. This includes public health activities such as cancer registry reporting; reporting suspected abuse or neglect; health oversight audits and investigations; responses to subpoenas and court orders; specified law-enforcement purposes; coroners, medical examiners and funeral directors; organ and tissue donation; approved research; averting a serious threat to health or safety; specialized government functions including military and national security; and workers’ compensation.
Uses that always require your written permission
We will not use or share your information for these purposes unless you give us written authorization:
- Most uses and disclosures of psychotherapy notes
- Marketing
- Sale of your health information
If you give us authorization, you may revoke it in writing at any time. Revoking it stops any future use or sharing; it cannot undo what has already been done in reliance on it.
Your rights
To see and get a copy. You may inspect and receive a copy of your medical and billing records, in electronic form if we hold them electronically. Ask in writing. We may charge a reasonable, cost-based fee.
To ask us to correct your record. If you believe something is wrong or incomplete, ask us in writing to amend it and say why. We may decline, and if we do we will explain why in writing and you may file a statement of disagreement.
To get a list of disclosures. You may request an accounting of certain disclosures we have made, other than those for treatment, payment and operations, for up to six years before the date you ask.
To ask us to limit what we use or share. You may request a restriction. We are not required to agree, except in one case: if you pay for a service in full, out of pocket, you may require us not to share information about that service with your health plan.
To choose how we contact you. You may ask us to reach you at a particular address or phone number, and we will accommodate reasonable requests.
To get a paper copy of this notice. Ask at any center, even if you agreed to receive it electronically.
To be notified of a breach. We will tell you if a breach compromises the privacy or security of your information.
Complaints
If you believe your privacy rights have been violated, tell us — write to the Privacy Officer at the address below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, or at hhs.gov/ocr.
You will not be penalized, and your care will not be affected, for filing a complaint.
Changes to this notice
We may change this notice, and any change applies to information we already hold as well as information we receive afterwards. The current notice is always posted on this page with its effective date, and a copy is available at every center.
